The ring of suffering (i) is an upgraded version of the regular ring of suffering. It can be imbued as a reward from the Nightmare Zone, costing 725,000 reward points or 362,500 with the hard tier of Combat Achievements, at Soul Wars for 300 Zeal Tokens, or by using a scroll of imbuing purchased from Emir's Arena for 200 points. Its stats are double those of the ring of suffering.

The ring can also be charged with rings of recoil to give it the recoil effect, renaming the item to ring of suffering (ri). Each ring of recoil provides 40 charges, assuming the ring of recoil has not been drained prior. A maximum of 100,000 charges can be stored, equivalent to 2,500 rings, at a cost of 2,047,500 coins. Players can configure the charges while the ring is in their bank by right-clicking on Configure-Charges, as long as players have the required materials. If a player uncharged the ring, the recoil rings stored will not be refunded. The ring of recoil effect is toggle-able, so players may disable it as to not waste it during training and instead use it in activities such as fighting Zulrah, or in PvP combat, where the effect may prove more useful. Charges are not lost upon death.

If not protected, dying to another player will cause the imbued ring to lose its imbued status, however the player will be reimbursed the Nightmare Zone points, Zeal Tokens or Scroll of imbuing to re-imbue it via the aforementioned methods. Dying to a monster will not remove its imbued effect.
