Ancient Wyverns are Slayer monsters that require a Slayer level of 82 in order to be harmed. They reside in the Wyvern Cave on Fossil Island and are the strongest species of Wyvern in Old School RuneScape. Like with other Wyverns, an elemental shield, mind shield, dragonfire shield, dragonfire ward or ancient wyvern shield provides significant protection against their icy breath, and they are vulnerable to the effects of dragonbane weapons. These monsters require completion of the Bone Voyage quest to be assigned as a Slayer task.

In addition to being harder to fight, Ancient Wyverns share the ability with Taloned Wyverns and Long-tailed Wyverns to use a typeless ranged attack that resembles a lighter coloured Water Blast. This attack ignores Protection prayers and hits for up to 10 damage, though it can also miss. They also use a Slash melee attack. When fighting them, the highest defensive armour the player can access is best suited to use against them. Should one choose to use ranged to fight them, it is recommended to fight in melee range and use Protect from Melee after de-aggroing the Wyverns.

Aggression can be removed by setting up a tolerance zone and staying within it.

The task-only cave entrance is a trapdoor in the Mushroom Forest and the regular cave entrance is south of the Museum Camp. When a player attempts to attack wyverns in the Slayer task area without a Fossil Island wyvern Slayer task, Weve (Steve's son) will stop them from doing so.

• When using their icy breath attack, the breath appears above the wyvern rather from their mouths.
• They appear to use all attack styles when using melee, depending on the animation they use: slashing when they use their talons, stabbing when they jab with their tail and crushing when they bite the target.
• Ancient wyverns had a festive override wearing a yellow partyhat during the 2017, 2018, and 2019 Christmas events.
