The Soulreaper axe is a two-handed axe that requires 80 Attack and Strength to wield. It is created by combining four untradeable components, obtained as drops from The Leviathan, The Whisperer, Vardorvis, and Duke Sucellus, alongside 2,000 blood runes and level 75 Magic (which cannot be boosted).
Despite being made from items dropped by prominent Zarosians, the item itself does not grant god protection towards Zarosian NPCs in the God Wars Dungeon.

Soul stacks
Soul stacks are generated every time a player makes an attack and has not reached five stacks, which are applied whenever the player attacks with the axe (even if they miss). Each stack is added after the damage from the player's attack is calculated, so an attack does not benefit from the 6% bonus it just generated.
Each Soul stack provides a 6% boost to a player's Strength level, up to a maximum of +30% at five stacks. This boost is additive with, and works similarly to, the Strength multipliers from prayers like Piety or Ultimate Strength. Soul stacks do not affect other weapons. Each stack will last for 50 ticks if the player is unable to attack during that time frame.
When the player can maintain five stacks for the 30% strength bonus, the soulreaper axe has the capability to deal more damage than weapons like the inquisitor's mace or blade of Saeldor, and can even out-DPS the scythe of vitur against 1x1 or 2x2-sized monsters. Against 3x3 or larger sized monsters, the scythe of vitur will typically provide a higher damage output. The boost provided from the soul stacks can be further increased with boosts such as Super strength potions and strength-boosting gear.
Soul stacks are maintained as long as a player is actively attacking with the Soulreaper axe equipped, with one stack decaying every 50 ticks (30s) if the player is not doing so. In areas with aggressive enemies, very little input is required to keep the strength buff maximised. This makes the axe exceptional for combat encounters with little downtime, including bosses like Araxxor and Vardorvis, and lower intensity combat like Slayer and the Nightmare Zone.
Special attack
The Soulreaper axe has a special attack, Behead, which consumes all Soul Stacks and does not require special attack energy.
For each Soul Stack consumed, Behead increases the player's accuracy by 12% and maximum and minimum damage by 6%, meaning when using a special attack with all five stacks, the attack will have 60% increased accuracy, 30% increased max hit, and deal a minimum of at least 30% of their max hit.
Equipping the Soulreaper axe replaces the special attack interface with a unique graphic () that indicates the Soul stack resource, however player's special attack energy continues to regenerate in the background normally. Since the special attack only consumes Soul stacks, in PvP it's useful to combo Behead into special attacks from other weapons like the granite maul, dragon claws, or voidwaker for high amounts of burst damage on the target.
Mechanics
Although the in-game description states Behead deals additional damage, this is in relation to base damage and not the boosted level from active soul stacks.
When using Behead, the Strength boost from Soul stacks is consumed first and a new separate damage bonus is applied to the swing. Consequently the strength bonus is then lost, meaning Behead also costs 90% of an additional attack when a player continues combat.
Note: in rare situations, the alternate damage calculation of the special attack can result in Behead having a lower max hit than a regular auto attack.
The accuracy roll for Behead uses the attack bonus of the player's currently selected style, which is rolled against the corresponding defensive bonuses of an opponent – either slash or crush in this case.[1] This is in contrast to most other special attacks, which roll against the defensive bonuses of an opponent of a specific combat style regardless of the attacking combat style chosen (for example, the dragon dagger's special attack will always roll against the opponent's slash defence, even if the attack style is set to stab).
