Shadow tentacles are NPCs that appear after every standard attack by the Whisperer. Four will surface within a five tile radius from the player before converging on their location. If the tile is blocked by an obstacle, the tentacle will not spawn.

The quest and post-quest variant makes use of two formations; an X-formation and a +-shaped formation, the latter of which begins to be used after the second special attack. In the awakened fight, the X-shaped formation is instead enhanced to cover all but one random area, while the +-shaped formation is changed to six tentacles lined up next to each other, which begins to be used after the fourth special attack.

The quest variant deals 10 damage and drains 10% sanity if hit. The post-quest variant deals 20 damage and drains 20% sanity. Lastly, the awakened variant deals 30 damage and drains 30% sanity. The damage and sanity drain can only hit once, so getting hit by multiple tentacles in the same tick is the same as getting hit by one.

Shadow tentacles converge onto the player's location.
