Vardorvis' Head is encountered whilst fighting Vardorvis. His head was chopped off by dwarves who raided his camp after he was infected with the Strangler parasite.

During the fight with Vardorvis, the head will appear once he has fallen below 570 hitpoints; when it appears, a message will appear in the text box stating Vardorvis' head gazes upon you.... It will proceed to launch a green, cone-shaped projectile at the player, which can be blocked by using Protect from Missiles. If the player fails to block the projectile, they will lose 10 prayer points, disables any active overhead protection prayers and prevents them from being used for the next three ticks. The head will always attack off-sync with the body, allowing for them to be flicked. Upon hitting enrage (<231 health), the head will emerge at a more frequent rate.

In Vardorvis' awakened mode fight, the head will also utilise a blue, crescent-shaped projectile which is blocked with Protect from Magic. Upon reaching 50% of his health, the head will attack in pairs, with one using the blue projectile and the other green. The order is random, but the second head will always use the projectile opposite of the first. When enrage mode hits, the heads will emerge at an extremely rapid rate, which can result in four heads emerging within one axe cycle.

While the head will always attack off-sync with Vardorvis' own attacks to block them, missing a prayer will likely result in the player taking heavy damage from Vardorvis or his axes.
