The guardian thrall was an NPC that was summoned via the guardian horn, which was available for players who had unlocked the Guardian relic during the Raging Echoes League. When summoned, she would run to attack the same target that the player was attacking with a style that the NPC had the lowest defence bonus for.

The guardian thrall had an attack speed of 4 (attacking every 2.4 seconds), with a damage roll of 5-13 and an accuracy roll of 45,000. In multicombat areas, she would attempt to cast an area-of-effect attack if the primary target is weak to ranged or magic, which can be toggled via the guardian horn. Additionally, Arceuus thralls could be summoned alongside her.

The guardian thrall's outfit was able to be changed via the horn. If Varlamore had been unlocked, there was a small chance for the guardian thrall's appearance to change to Echo Cuthbert. When changed to this appearance, the player stated Rise, Echo of Cuthbert....
