Blighted anglerfish are a variant of anglerfish that can only be eaten while inside the Wilderness, Ferox Enclave, Mage Arena bank, PvP Worlds, Castle Wars lobby, and Clan Wars. 15 can be purchased at Justine's stuff for the Last Shopper Standing for 1 Last Man Standing point or at the Soul Wars Reward Shop for 10 Zeal Tokens.

Similarly to anglerfish, blighted anglerfish can heal above maximum Hitpoints (making the maximum Hitpoints possible 121). Overheal does not apply if the player has been attacked or damaged (including zeroes) inside a PvP area within the past 16 ticks (9.6s), whether from a player, monster, or environmental source. There is a small exception: when fighting an NPC in a single-way combat area, you can overheal as soon as the NPC dies rather than waiting 16 ticks.

When attempting to eat the anglerfish outside of a PvP area, the message The blighted anglerfish can be eaten only in the Wilderness. is displayed. Otherwise, the standard You eat the anglerfish. It heals some health. is displayed.
