The Whisperer's medallion is one of the four medallions used to access the lower level of the Ancient Vault during Desert Treasure II. It is obtained by searching the entrails of The Whisperer after defeating her.

As it cannot be deposited into a bank, it is recommended that players bring it to the Ancient Vault immediately, as the Mysterious Figure may appear and attack the player if they have it in their inventory for too long.

If you lose the medallion during the quest, you can recover it by returning to where you fought The Whisperer. The entrails will be there and you can grab another medallion without having to fight again.
