The broken coffin can be obtained from Dampe in the town of Mort'ton. The coffin can be upgraded by giving locks found from the Shades of Mort'ton minigame to Dampe. Any lock can be added to the coffin at any point; lower tier locks do not need to be added to the coffin in order to upgrade it to the next level.

You can only have one coffin at a time, i.e. if you already have one - either broken or with a lock attached - Dampe will refuse to give you another one.
