Wrath runes are the highest level runes in Old School RuneScape. Released alongside the Dragon Slayer II quest, they are used for surge spells, which are the strongest elemental spells in the standard spellbook, and offering spells in the Arceuus spellbook.

Wrath runes can be made with 95 Runecraft at the Wrath Altar by using pure essence or daeyalt essence on it, providing 8 or 12 Runecraft experience each, respectively. Scarred extracts can also be used to increase the quantity of crafted wrath runes by 60 per pure essence, requiring partial completion of Desert Treasure II - The Fallen Empire and access to the Scar essence mine.

Access to the Wrath Altar requires completion of the Dragon Slayer II quest. To enter the Wrath Altar, the player needs a wrath talisman or tiara, catalytic talisman or tiara, or a Runecraft cape. The Wrath Altar can be reached by entering the statue in the Myths' Guild and going west then south in the dungeon, through the blue dragons and entering the crevice in the southern wall, ending up on an island with the altar ruins.

Wrath runes cannot be obtained via the Ourania Altar and the altar cannot be accessed via the Abyss.

• The wrath rune's design alludes to both death and blood runes: the skull and black colour being associated with the death rune, and the crimson colour associated with the blood rune.
  • Originally, the surge spells in RuneScape 3 required one death and one blood rune along with their respective elemental runes to cast. This was not implemented in Old School RuneScape, as the wrath rune was introduced instead in order to cast the spells.
• The option to craft wrath runes via the Abyss was polled alongside Dragon Slayer II, but failed with 73.6% "Yes" votes.
