A potion is given to a player by asking the Apothecary in Varrock for a free sample. The Apothecary will give it roughly half the time, otherwise refusing to do so. Multiple potions can be obtained using the drop trick. It can also be obtained as a rare drop from killing Imps, or failing The Gauntlet.

The Apothecary indicates that the potion is a spot cream. If used on various citizens in Sophanem before completing Beneath Cursed Sands, it is revealed that the potion is used to relieve itching.

This potion has only a few uses:

• It is a possible item requirement for the Forgettable Tale of a Drunken Dwarf quest if the rowdy dwarf requests for the player to bring them an acne potion.
• It is also a possible item request for Keldagrim tasks given by the rowdy dwarf and Gunslik.
• It can be used on the civilians of Sophanem who are covered in spots prior to the completion of Beneath Cursed Sands which results in various amusing conversations. Citizens with different dialogues are as follows: Priests, Workers, the Embalmer, Siamun, and Raetul (shared with Carpenter).
