The Medallion of the Deep is a neck-slot item which allows the player to dive underwater without the need for a diving apparatus and fishbowl helmet. It is created by assembling eight medallion fragments found on various islands accessed via the Sailing skill, granting 750 Sailing experience along with 500 Crafting experience.

Assembling the medallion requires that the player read the waterlogged journal, which was written by the medallion's creator and contains hints on the location of the fragments. As the final fragment is found on Wintumber Island, the minimum Sailing level needed to obtain the medallion is 63.

If the medallion is lost, players can reclaim it by searching the crate where the waterlogged journal was found (near Henderson on the Pandemonium). Upon doing so, a chatbox message will appear, stating "Against all common sense, the Medallion of the Deep has somehow found its way into this crate. You quickly grab it before you lose it again." Another medallion can also be obtained from the crate after one has been stored into a STASH unit on Brittle Isle.

• Prior to 20 May 2026, if the player assembled the medallion while in the Underwater Agility and Thieving area, Mairin forbade them from leaving via the normal exits, treating the player as if they had a fishbowl helmet in their inventory. The only way to leave was to either drown, teleport out, or destroy the medallion and leave via the normal exit.
