Breaches are spawners in Deadman Mode which shoot out projectiles spawning breach monsters. In permanent Deadman Mode there are two types of breaches, one that is simultaneously in two locations, of which one is in single-way combat and one in multicombat. The other type is a region breach in which there is only one breach spawner but the projectile range spans more widely.
When a breach spawns, it lasts for 15 minutes. Over the course of those 15 minutes, it will spit out projectiles at the surrounding area, and a Boss Spawn will appear where the projectiles land, which spawns a breach monster.
The player can find the location of the active breach by talking to Perdu or using the Deadman's skull. The skull can also be used to check how long until the next breach occurs.
