The bottomless milk bucket is dropped by Brutus, the repeatable post-quest boss of The Ides of Milk quest. It can be used to hold up to 10,000 uses of a variety of milks and milk-related products, albeit one at a time:

If a player has multiple bottomless milk buckets, each bucket can hold a different type of milk. These can be used in quests such as Gertrude's Cat, Skippy and the Mogres, and Plague City; a single charge will be used from the bottomless milk bucket rather than emptying it entirely. However, a bottomless milk bucket filled with chocolatey milk cannot be drunk to replenish health, and a bottomless milk bucket filled with milk cannot be used in place of a regular bucket of milk to build a pet feeder. The bottomless bucket also cannot be used on a regular bucket to decant milk despite being able to use a regular bucket of milk on a bottomless bucket to add charges to it. Similarly, the bottomless milk bucket can not be used in the Gnome Restaurant Minigame to create orders. The bottomless bucket cannot be consolidated with the contents of another bottomless bucket.

It is possible to fill the bottomless milk bucket directly from a dairy cow by milking the cow while having the bottomless milk bucket in inventory, however it is not possible to milk a dairy buffalo with the bottomless milk bucket. The process is automatic, requiring the player to only click once to start milking, after which one charge is added to the bucket every 7 ticks, or 6 ticks if wearing the cowbell amulet, until the bucket is full or the player is logged out due to inactivity. Filling the bucket completely takes approximately 11 hours and 40 minutes without the cowbell amulet, and 10 hours while wearing the cowbell amulet.

A bottomless milk bucket filled with milk can be used at a dairy churn to produce cream, butter or cheese. The latter of which has an AFK timer of 7 minutes and grants over 15,000 cooking exp per hour. This is the cooking skilling method with the highest AFK timer.

Unlike the bottomless compost bucket, the bottomless milk bucket is untradeable even when empty. Although the bottomless bucket has a risk value of 100,000, if you die with the bucket in the Wilderness beyond level 20, the player that kills you receives 5,400 coins.

• Filling the bucket completely displays a message of "Your bottomless milk bucket, surprisingly, is full." Attempting to milk a dairy cow with a filled bucket results in the same message.
