Monstrous Basilisks are larger variants of basilisks that are encountered during the The Fremennik Exiles when basilisks attack in an attempt to take over Rellekka. Defeating one adds 10% towards the completion of defending the region. Like other basilisks, they have a gaze ability that drains all combat stats, so defeating them requires a mirror shield or V's shield, which is collected shortly before the invasion.

Despite being based on the Superior slayer monster of the same name, players only receive 170 experience in Slayer when killing a monstrous basilisk during the quest, which is one tenth of killing a regularly-spawned one during the respective Slayer task.

• The examine message is a reference to Harry Potter and the Chamber of Secrets. The only antidote to the venom of the basilisk was the tears of a phoenix.
