Basilisks can be assigned as a Slayer task at level 40 Slayer, level 40 combat, and level 20 Defence by medium level Slayer Masters. The high level masters will not assign them until players unlock Basilocked using Slayer reward points. Ordinary basilisks, which can be found in the Fremennik Slayer Dungeon and Jormungand's Prison, attack only using an extremely inaccurate melee attack with a max hit of 5. Their thin drop table and high defence level generally make them worthwhile to skip, although their low weighting doesn't make them worth blocking.

However, after completion of The Fremennik Exiles quest, the player gains access to the Basilisk Knight in Jormungand's Prison, and basilisk tasks gain a new purpose for obtaining the powerful Neitiznot faceguard.

When fighting any basilisk, the player must equip a mirror shield or V's shield, or the basilisk's gaze will deal immense damage and massively drain the player's stats.

The knights have both a melee attack which can hit up to 20 and a magic attack with the same max hit. They also have a unique mechanic where they will match the player's Attack speed with a max frequency of four ticks. Knights will exclusively use their magic attack at distance while attempting to move into melee range. Protection prayers can block each kind of attack, although not simultaneously.

While not on a basilisk task, Basilisk knights drop the Basilisk jaw, which is combined with the Helm of neitiznot to create the Neitiznot faceguard, at a rate of only 1/5000. While on task, however, this increases to 1/1000. Therefore, on average, Basilisk knights will deliver a drop worth 15,260,475 every four to five Extended tasks. Due to their high defence these tasks can be relatively slow, with a maximum possible kill rate of only 100 per hour. The task unlock for Konar, Nieve, and Duradel to assign basilisks can be toggled off once the player receives their jaw, but requires paying Slayer points to activate again.

Both variations of basilisk have their own Superior slayer monster variation. Ordinary basilisks may spawn the slightly stronger Monstrous basilisk, while knights may spawn the highly dangerous Basilisk Sentinel. The sentinels also have a special attack which launches a slow-moving projectile at the player. The player must either move one tile to avoid it, or be encased in stone. If encased in stone, the player must click rapidly to escape or else receive massive damage.

Knights have a reasonable drop table when including the jaw, but a poor one without. Still, they frequently drop alchable equipment, dozens of mid to high level Catalytic runes, and occasional high level herbs.

Cannons cannot be placed anywhere with any kind of basilisk.
