Gnomeballs are received by winning 5 points in a game of Gnome Ball, from the gnome restaurant minigame, or as a drop from dolphins. With the item in your inventory, you can click "Use" with it, and click another player. If that player has their right hand empty, they will catch the ball, and it will end up in their weapon slot. It is not advised that you throw it to a player you do not know, as they can very easily walk away with it.
Gnomeballs cannot be used on or by Ironman Mode players.

Trivia
- Attempting to bank a gnomeball with an Imp-in-a-box will result in a message box appearing with the imp in the box saying: "I'm not touching a gnomeball; that's a vicious game! I might die!"
- The gnomeball has been used by many players to determine whether or not an account is botting, as the sudden appearance of a foreign object in their inventory may halt their programming.
- If a gnomeball is dropped in the Gnome Ball minigame, the gnomeball will not appear on the ground and instead results in the message .
- Attempting to go Underwater or to the Coral Nurseries with a gnomeball equipped displays the chatbox message "The gnomeball you are carrying feels very buoyant. Perhaps you should leave it behind."
- Attempting to go diving with the gnomeball in the inventory will result in Murphy asking if the player has a gnomeball on them and then telling the player: "Well you will need to leave them on the surface. They are so full of air that they will stop you diving properly."
