The top of sceptre is a part of the runed sceptre and eventually the skull sceptre. It is dropped by Catablepon on the third level of the Stronghold of Security. Combining it with the bottom of sceptre makes the runed sceptre. Using it with a chisel makes bone fragments, 3 if none of the Varrock Diary is complete and 4, 5, 6, or 7 respectively depending on which tier of the diary is complete. Bone fragments can be added to a Skull sceptre (i) to add one charge per bone fragment.

After combining a message in chatbox will state: "The two halves of the Sceptre fit perfectly. The Sceptre appears to be designed to have something on top." The Stronghold of Security is the dungeon beneath Barbarian Village, to the west of Varrock and east of the Dwarven Mine.
