A smouldering heart is a rare untradeable item dropped by tormented demons. It is dropped on the same loot table as the smouldering gland and the smouldering pile of flesh, and acts as a one-use consumable item; unlike most items, which have a "Take" option on the ground, the Smouldering heart has a "Crush" option, and cannot be telegrabbed.

Crushing it releases energy from the heart, providing a significant stat boost to all combat stats; at level 99, the player receives a +20 level boost to Attack, Strength, and Defence, +14 to Ranged, and +11 to Magic. The smouldering heart energy will remain at their respective maximums for two minutes, after which they start decaying naturally. Note that a divine potion effect ending will reset the related stats as if they were part of the divine potion boost, and its effects carry over upon death.

Upon crushing the heart, a game message in the chatbox will state You crush the heart. Dark energy swirls around you... Players have ten minutes to crush the heart before it disappears from the ground; this timer is paused upon logging out.

• The ground item model and its corresponding graphical effect when crushing the heart are identical to the imbued heart, but recolored. While the activation of the imbued heart affects only magic, activating a Smouldering Heart grants bonuses to all combat styles.
• The magic level boost gained from the Smouldering Heart can be used as a boost to access higher level enchant spells, with level 83 being the earliest to boost for Zenytes.
