Respiratory Runner is an elite combat achievement which requires the player to kill the Abyssal Sire after only stunning him once.
This task fails whenever Sire wakes up from a stun; killing the respiratory systems via safespotting or tanking the tentacles must be done before stunning Sire. Likewise, enough damage to the Sire must be dealt or a shadow spell cast to disorient it exactly once.
Leaving the nexus will fail the achievement attempt unless you don't wake up the Sire.
Strategy
The scorching bow trivialises this achievement, as it will always kill the respiratory systems in one shot, regardless of Ranged accuracy and ammo used.
For players without a scorching bow, note that upon a successful hit, players will always hit at least 50% of their maximum hit on the systems, meaning a setup with a maximum hit close to 50 will always kill the systems in 2 shots. Additionally, the bonuses from a Slayer helm apply to the respiratory systems. With high levels in either Magic or Ranged and late-game gear (such as a bow of Faerdhinen and full crystal armour), it is possible to kill all four in a single stun, but it will require minimising the time taken to travel between systems, such as running to the next system immediately after the player receives an experience drop that confirms they killed the previous system.
A Magic or Ranged weapon that can fire at a range of 10, including those that can use the longrange attack style, can be used to safespot the respiratory systems in the south-west and north-east of each boss chamber without the tentacles being able to attack. To do this, wake up Sire but do not stun it. Safespot the first two respiratory systems, then stun Sire and quickly kill the remaining systems before Sire can wake. If three systems can be killed in the duration of one stun, then only one system need to be safespotted.
Alternatively using thralls; either Resurrect Greater Ghost or Resurrect Greater Skeleton from the Arceuus Spellbook, can slowly, but reliably take down the vents, even when the Abyssal Sire is not disoriented and without using shadow spells.
