The rotten carrot is an item available only to Jagex Moderators, similar to the rotten potato, egg and cabbage.
About and uses
Using the rotten carrot on an account will instantly kill them and show an interface with the account's bank and inventory contents, which will be teleported to the user's bank.
It is used for the Bot Busting stream on the Old School RuneScape Twitch channel, which began at 8pm BST/3pm EST on Saturday June 16th, 2018.
