The doctor's hat is used during A Tail of Two Cats to convince Unferth that you are a doctor. It is obtained from the Apothecary.

If a player gets a nurse hat during the quest and later wants to get the doctor's hat, or vice versa, they can destroy their current hat and talk to the Apothecary to choose which hat they want. Players cannot own both hats at the same time.
