Cat ears are a head-slot cosmetic obtained as a reward from the 2020 Birthday event. They have a right-click 'chase' option which allows players to chase Hell-Rats, which will turn them into hell cat ears.

If destroyed, a replacement can be obtained from Diango in Draynor Village or from the toy box of a costume room.

• Attempting to pet or shoo-away a stray dog, chiribaya, or Molossus while wearing them gives the message The dog doesn't approve of your ears., after which the dog will flee.
• Talking to a pet cat with them equipped gives the message "UwU?", and more unique dialogue if wearing a Catspeak amulet
• Talking to the Sphinx enables a string of dialogue involving cat puns.
• Talking to Gertrude with the cat ears equipped gives additional dialogue where she thinks the player is a cat and offers them a sardine.
