I should see a doctor is a grandmaster combat achievement which requires players to get kill credit for Nex while a player is infected with Nex's Choke attack.
Strategy
For the task to be completed, players must purposely spread the virus to other players for the entirety of the fight. Players will cough four times (three with a slayer helmet) for each cough, which resets if they infect another player with it. The best way to complete the achievement is to have two or three players purposely infect each other while the rest of the team takes down Nex. These players should ideally not engage Nex in battle to avoid spreading the virus to other players who will be killing Nex.
