The Trapdoor is located in the norwestern-most section of the Grand Exchange. It can be interacted with to travel to Keldagrim via minecart once the player has started The Giant Dwarf quest.

Inspecting the trapdoor will start a dialogue with a cart conductor which is influenced by whether or not the player has a follower out.
