A lever bracket is a broken lever located in the Temple of Ikov, on the south-west wall from the leader near the entrance. The lever has to be repaired before it can be used, by using the lever piece on the bracket. The lever can then be pulled.
Warning: If you place the lever but do not pull it and then leave the area, the lever will not be there when you return. You will have to cross the bridge again to collect it.
It only appears in that dungeon during the Temple of Ikov quest.
